 2 years  Hib#4 and/or PCV13 if not already given
 3 years  Hib#4 and/or PCV13 if not already given
 4 years  Hib#4 and/or PCV13 if not already given
 5 years  DTaP#5  IPV#4  MMR
Varicella
 11/12 years  TdAp  Menactra  Gardasil opt. (1st of 3 doses given  over 6 month period)
 18 years  Menactra  TdaP opt.

 HepA=hepatitis A  Pentacel=DTaP/Hib/IPV
 HepB=hepatitis B  DTaP=diphtheria, tetanus, acellular pertussis
 IPV=polio  Hib=haemophilus; Pneumo=pneumococcal
 Varicella=chicken pox  MMRV=measles, mumps, rubella and varicella
 Rotateq=oral Rotavirus vaccine
 Gardasil=HPV vaccine for girls age 9 and older
 Menactra=meningitis vaccine  
 PCV13=pneumococcal-13
